We massage therapists need all sorts of different forms and templates. I enjoy designing customized forms for my business. These are the most common ones I get requests to share. Most customizations can be done in an hour or two tops. Edits for customized forms can be made, but don't get crazy. Keep the number of edits down by providing all the information you want on your form.
